Our team of nurses, home care aides, therapists and social workers can provide:

Developing the Plan
One of our registered nurses will consult with your physician and meet with you and your family members to tailor a plan of care that best fits your needs. This will include a full general assessment in your home or place of residence at no charge. You will also have the chance to ask any questions you many have before your service begins.

Intermittent Home Visits provide the patient with skilled services such as nursing or therapy on a part time basis for a specific period of time. Medicare, Medicaid and private insurance may pay for this type of care if the patient meets certain requirements.

Private Home Services provide care to patients who need assistance up to 24-consecutive hours. Services are provided by skilled, dependable professionals which can include nurses, therapists, patient care technicians and companions.

Medical Social Workers are able to assist the patient and family members dealing with the difficult issues of prolonged or chronic illness. They also provide access to community resources that can assist the patient in remaining at home.

Occupational Therapy, Physical Therapy and Speech Therapy are all available in the home setting allowing the patient to recover in the privacy of their own home.

Pre-Discharge Consultation provides the patient, family and physician an opportunity to establish an individual plan of care, discuss expectations and answer any questions.

Patient/Caregiver Education is an important part of the home care experience. The patient takes a very active role in their healing process.
